Required Documents ...to be submitted to the Department of Financial Services (as Registered Agent):
- The original Validated Summons or Subpoena
- Two copies of Summons or Subpoena
- Two complete copies of the complaint including any attachments, exhibits or discovery that you wish to be served
- A separate check for $15 payable to the Department of Financial Services for each defendant that we are to serve.
Required Documents ...to be submitted to the Secretary of State (as Registered Agent):
- The original Validated Summons or Subpoena
- Two copies of the Summons or Subpoena
- Two complete copies of the complaint including all attachments, exhibits or discoveries you wish to serve
- The Florida Statute number for which you are serving the Secretary of State as the Registered Agent
- A separate check for $8.75 payable to the Secretary of State for each defendant being served
Helpful Hints:
Many insurers have very similar names and companies periodically change their official name. Please verify that the name of the defendant we are to serve is shown on the subpoena or summons and complaint exactly as they are registered with the Department of Financial Services or the Secretary of State.
